How do internal drop filters work?

Excuse my ignorance as I've always used internal & external canisters, but how do internal drop filters work? I've heard they're the tool of choice because of their huge capacity and low cost to run.

Anyone have a diagram?

Do they make much noise?

Apparently an airstone is used - do you necessarily need to use a pump?

Can you run it the opposite way, with the intake at the bottom and using a pump?

I'm sure this topic has been covered before but it's been a bit hard to glean info from searching the forum.
